Rates (Private Pay)
$150 per 45 to 50 minute session
$175 for Intake (first ) session (1 hour)

Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan (including EAP). Please check your coverage carefully by asking the following questions:

• Do I have behavioral health insurance benefits?
• What is my deductible and has it been met?
• What is my co-pay for each session?
• How many sessions per year does my health insurance cover?
• What is the coverage amount per therapy session?
• Is an authorization number required by my insurance carrier?
• Is approval required from my primary care physician?

Reduced Fee
Reduced fee services are available on a limited basis for private pay clients.

Payment
Cash, check and all major credit cards accepted for payment.

Cancellation Policy
If you cancel your appointment or do not show up for your scheduled therapy appointment, and you have not notified my office at least 24 hours in advance, you will be required to pay a $25.00 cancellation fee.
